Eglin Air Force Base Collaborates to Create a Patient-Centric Health Care Model

The 96th Medical Group (96th MDG) at Eglin Air Force Base is actively participating in the development of a new, patient-centric health care model in collaboration with other Defense Health Agency (DHA) medical facilities. The aim of this innovative model is to transform the way patients access care and how providers deliver it.

This collaborative effort will enable the DHA to enhance the current health care model and expand it to the entire military health system by the end of this year.

Sustainable Development Goals (SDGs)

The implementation of this new health care model aligns with several Sustainable Development Goals (SDGs), including:

The initial phase of My Military Health implementation introduces a digital self-care wellness program called SilverCloud. This platform provides on-demand access to clinically-validated, self-paced, private wellness programs and coaching support to enhance resilience and improve the well-being of patients.

In addition to SilverCloud, patients will soon have access to other digital platforms and resources offered by My Military Health, including:

  • Virtual Visits: This feature allows patients to schedule video and in-person appointments, receive appointment reminders, conduct online searches for providers, and access virtual waiting rooms.
  • Care Companion: This resource improves health care plan compliance and helps individuals stay connected with their health care team between appointments.
